Who Qualifies

Understanding Hospice Eligibility

Hospice care is a Medicare and Medi-Cal benefit available to patients with a life-limiting illness. You don't have to wait until the very end — earlier enrollment often means more time for comfort and connection.

A physician has determined a life expectancy of 6 months or less if the illness follows its natural course

The patient chooses comfort-focused care over curative treatment

Diagnosis includes cancer, heart failure, COPD, dementia, ALS, or other serious illness

Patient is enrolled in Medicare Part A, Medi-Cal, or qualifying private insurance

Care can be provided at home, in a skilled nursing facility, or assisted living

Family members and caregivers are included in the care plan at no additional cost

For Families

You don't have to navigate this alone

We guide families through every step — from the first conversation to bereavement support. Our team is here to answer questions, ease fears, and help you make the most of the time you have together.

Our Philosophy

Rooted in community. Present in every moment.

Dignity above all

Every person deserves to be seen, heard, and honored — regardless of diagnosis, background, or circumstance. We enter each home as guests, and we leave it better than we found it.

Families are part of the team

We don't care for patients in isolation. The people who love them are central to everything we do — included in care planning, educated at every step, and supported long after loss.

Presence over procedure

Clinical excellence matters deeply. But so does sitting quietly with someone who is afraid, or calling a family member just to check in. We measure our work in moments, not metrics.

Giving back to our community

Aceso Hospice is proud to serve alongside the first responders, volunteers, and neighbors who make Ventura County, Los Angeles, and Santa Barbara the communities they are. Caring for our community is not just our mission — it is who we are.
